Our Programs

One-on-One Diabetes Self-Management Training

Our personalized sessions focus on the health priorities of each individual patient. During the initial assessment, your learning needs are identified and subsequent sessions are scheduled as needed, based on your behavior goals and clinical outcomes.

One-On-One Nutrition Therapy

Medical Nutrition Therapy (MNT) is provided through the wellness component of the practice. Self-management education is provided to patients for various nutritional diagnoses including obesity, renal, and cardiac disease.

Diabetes Self-Management Education

We offer interactive group classes that focus on weight-management and diabetes prevention. Our ADA certified program teaches participants to develop comprehensive strategies for maintaining day-to-day health. Please contact us for next availability.

Meet The Team

Elizabeth integrates 40+ years of clinical experience as a Registered Dietitian Nutritionist with an additional certification as a Diabetes Educator and Lifestyle Counselor. By integrating contemporary knowledge in behavioral science, expertise in diabetes self-management education and medication protocols, she is highly effective in teaching groups and individuals how to manage their disease. She is equally adept in developing outpatient nutritional regimens for patients presenting with various chronic medical conditions.

Juliana King is a graduate of San Jose State University where she earned a Master’s degree in Nutritional Science and is a registered dietitian nutritionist. She has experience educating individuals both in patient-centered and corporate environments in the Bay Area and is currently working towards becoming a certified diabetes educator.

Our diabetes care coordinators provide valued client services and are available during business hours to answer questions, make appointments and facilitate insurance claims. They are trained to take biometric measurements of patients on initial visits to our office, as well as communicate with medical offices in a professional and friendly manner.

Our student volunteers are talented future dietitians and diabetes educators. The Bay Area Diabetes and Wellness practice gives them the opportunity to gain valuable experience and the potential to start applying their academic learning in a professional environment.

*For those PPO plans that we are not currently contracted with, you may use your Health Savings Accounts or Flex Spending Accounts to optimize your tax benefits.  As a courtesy, Bay Area Diabetes and Wellness Center will submit your claim to your insurance.  You will receive an estimate of benefits (EOB) once the claim is processed.  What is spent at this practice qualifies as your annual deductible.  Once the deductible is met, your insurance will directly reimburse you with a check for the allowable amount.

Medicare Coverage

There is no coinsurance if Medical Nutrition Therapy service is provided.

There is 20% coinsurance on Diabetes Self-Management Education service. Current Medicare law allows only 2 diagnoses to be covered for payment: diabetes and kidney disease (stages 3-5 before dialysis and post transplantation).
